Child Participation Form

Please complete this form for any person's under 18 attending any Flying Dove Theatre classes, courses or workshops.

The form must be completed by a parent/guardian.

It is your responsiblity to update us of any changes that may occur to the information provided on this form.

Your child will not be able to participate in Flying Dove Theatre activites if the form is not completed.
